To create a new incident, please fill in the areas required (*) for us to conduct our investigation. Please include all details of your report with your LOCATION, DATE, TIME, PEOPLE INVOLVED and any other details that will assist us in our investigation. We will make every effort to keep your information reported CONFIDENTIAL and ANONYMOUS.
Once you click on "SEND" you will receive a confirmation that your report has been successfully transmitted and a report number will be given. If you would like to track the status of your report, please write this number down to track your incident reported. This number is encrypted for the sole purpose of keeping your information confidential. If you lose this number, we cannot replace this number for you. It is also recommended that you supply your telephone number for us to contact you back regarding any additional information needed. But keep in mind that you do not need to identify yourself.
